I am a participating provider for some insurance companies; for others I am considered an "out-of-network" provider. If you contact your insurer, these are the questions to ask:
- Do I need a referral from a physician before I can get therapy?
- Does my insurance cover a provider outside of network?
- What is my deductible that must be met first?
- How much will my plan cover for each session? (specific dollar amount? or percentage of fee?)
- Is there a limit on the number of sessions per year or on an amount of fees per year?
I bill certain companies for you directly; for others I will provide you with a receipt that contains all the information you would need to file a claim. If I am not an "in-network" provider for your insurance, I do ask that you provide payment at the time of service. I accept cash or checks. The fee for an initial appointment is $225, and ongoing appointments are $150 for a 45 minute session and $185 for a 60 minute session. I am a provider for the following insurance companies:
